Sleepless Nights: How Night Noise Is Undermining Public Health

Excessive noise is more than a nuisance; it is a health risk and an important determinant of health. As urbanisation, commercial activity and transport intensify, night-time noise is becoming a growing public health concern. It can disrupt sleep, affect wellbeing and contribute to wider environmental pressures.

Understanding Night Noise Pollution

The World Health Organization (WHO) identifies sleep disturbance as the largest contributor to the burden of disease from environmental noise [i]. Night-time exposure is commonly assessed using Lnight, the average outdoor noise level over an eight-hour night-time period across a year [ii]. WHO recommends keeping average night-time noise below 45 dB for road traffic, 44 dB for rail traffic and 40 dB for aircraft noise to prevent adverse health outcomes [ii]. In Malta, the Environment and Resources Authorityโ€™s (ERA) most recent Noise Action Plan identifies road traffic as the main source of night-time environmental noise [iii][1].

Leisure and entertainment noise create distinct challenges compared with road, sea or air traffic noise [iv]. WHO conditionally recommends limiting yearly average exposure from leisure sources to 70 dB, but this mainly addresses hearing impairment rather than sleep disturbance [ii]. Setting sector-specific environmental night-time limits is unsuitable because leisure noise is often discontinuous, concentrated on weekends and marked by high-intensity peaks that Lnight can mask, underestimating the burden on residents [iv].

The Public Health & Environmental Impact of Night Noise Pollution

Night-time noise is often dismissed as a nuisance, yet it can activate the bodyโ€™s stress response even without fully waking a person, increasing heart rate and blood pressure [v,vi]. Repeated sleep disruption is associated with a higher risk of non-communicable diseases, including hypertension, ischaemic heart disease and stroke [vii,viii]. Across Europe, transportation noise is estimated to contribute to 49,000 new cases of cardiovascular disease and 23,000 new cases of type 2 diabetes annually [ix].

Continued exposure to night-time noise pollution also takes a toll on mental health and is linked to depression, anxiety and reduced mental wellbeing [vi]. For residents affected by nightlife noise, feelings of irritability, nervousness, stress and a profound sense of despair further exacerbate adverse mental health outcomes [iv].

Chronic night-time noise exposure can affect health across the life course. Emerging evidence links chronic noise exposure with increased dementia risk in adults [vi], while children may experience poorer reading comprehension and behavioural difficulties [ix].

Furthermore, the impact of noise pollution extends beyond human health and can also disrupt wildlife communication, migration and feeding patterns. Across Europe, at least 29% of Natura 2000 protected areas are heavily affected by harmful transport noise, while underwater noise from shipping and marine construction can adversely affect many marine species [ix].

A Focus on Impulse Noise

Impulse noise differs from continuous noise in definition, measurement and health impact. It involves a rapid rise and fall in sound pressure lasting no more than 500 milliseconds, with intensity at least 15 dBA above background levels [x]. WHO guidelines note that usual noise metrics cannot be applied to impulse noise because the ear responds non-linearly at high sound pressure levels [ii]. At equivalent energy doses, impulse noise is associated with more frequent and severe hearing loss than continuous noise [xi,xii].

Examples of impulse noise sources include hammering, blacksmithing, forge work, automobile bodywork and industrial impact processes [xi]. WHO guidelines add shooting ranges and firecrackers as other examples [ii].

Impulse noise is characterised by microarousals, a brief 3-to-15-second wake intrusion into sleep that can be identified using electroencephalography (EEG) [xiii]. Microarousals are part of healthy non-rapid eye movement (NREM) sleep dynamics, but too few or too many may impair sleepโ€™s restorative function [xiii]. Recent WHO-related noise research more often refers to additional awakenings, showing that a 10 dB increase in indoor noise metrics raises the odds of awakening from aircraft, road and rail noise [xiv].

Public Health Interventions & Recommendations

Individuals can also reduce the impact of night-time noise through [v,vi,ix]:

  • Home modifications, such as relocating bedrooms away from noise sources or installing soundproof windows and doors
  • Low-volume masking strategies using white noise or natural sounds
  • Healthy sleep and stress-management behaviours
  • Advocacy to responsible entities for wider community-level interventions

Although these individual actions can help, the most effective public health strategy remains source control through regulation, enforcement, robust urban planning and upstream societal measures. A health-protective approach should ensure that residents can sleep safely and comfortably with their bedroom windows open [xvii,xviii]. This is especially important in Malta, where climate projections indicate that summer heatwaves are likely to become more frequent and intense, making reliance on closed windows and air-conditioning an inadequate and inequitable response to night-time noise.

Night-time noise pollution should not be treated as an unavoidable inconvenience, but as a serious public health concern. Its effects on physical health, mental wellbeing and the natural environment reinforce the need for an integrated approach that recognises the interdependence of people, animals and ecosystems.
